Later Life Transitions
Retirement Transition
Who it’s for: Adults navigating the shift out of the workforce, managing loss of structure, identity, and daily purpose alongside the freedoms of retirement.
We help clients redesign daily routines, reconnect with meaningful activities, and build a life after work that feels intentional rather than unmoored. This is one of the most significant occupational transitions a person can face — and one that rarely gets the clinical attention it deserves.

Chronic Illness & Energy Management
Who it’s for: Adults managing energy-limiting conditions alongside the occupational shifts of aging, including partial retirement, reduced capacity, and identity changes tied to health.
Energy conservation and pacing tailored to later life, activity modification, support for the work-to-retirement transition, and strategies for maintaining meaningful participation when health is variable.

Caregiver Support
Who it’s for: Adult children, spouses, and family members supporting an aging or ill loved one — navigating system complexity, caregiver burnout, and maintaining their own occupational balance.
We support caregivers directly, not only as a resource for the person they’re caring for. This includes caregiver burnout assessment and management, help navigating healthcare and community systems, strategies for sustainable caregiving routines, and support for the caregiver’s own identity and meaningful occupation.

Cognitive & Functional Changes (early stage)
Who it’s for: Older adults still living independently who are noticing subtle changes in memory, processing, or daily function — and want practical strategies to stay engaged and capable.
Rather than a deficit-focused approach, we work alongside clients to adapt routines, simplify environments, build compensatory strategies, and maintain the activities that matter most.

ADHD Functional Impact Assessment
A structured occupational therapy assessment evaluating how ADHD affects daily functioning, occupational performance, and participation across work, home, and community contexts. Using the Canadian Occupational Performance Measure (COPM) and structured clinical interview, this assessment documents the functional impact of ADHD on your client’s roles, routines, productivity, and quality of life.
The written report can be used to support workplace accommodation requests, academic accommodation letters, disability insurance documentation, and return-to-work planning. It is a functional OT report: it does not diagnose ADHD and is not a substitute for a psychological or psychiatric assessment.
Who it’s for: Adults with a confirmed or suspected ADHD diagnosis who need a functional report for workplace accommodations, insurance, or return-to-work purposes.

How it’s delivered: Virtual or in-person · By self-referral or professional referral.
Activities of Daily Living (ADL) & Independent Living Assessment
A comprehensive in-person assessment evaluating a client’s ability to manage personal self-care, home management, meal preparation, medication management, and community participation. Identifies functional strengths, areas of difficulty, and environmental factors affecting independence. Includes a written report with prioritized recommendations for supports, adaptive strategies, or environmental modifications.
Who it’s for: Older adults, individuals with chronic illness or disability, or anyone whose ability to live independently has been affected by a health condition or life transition.
